I give consent and authorize PFI Mega Life to use my personal data for administrative purposes, and services associated with the offering of this product/program and or other products/programs from PFI Mega Life Insurance.
Group Term Life Insurance product that protects the Insured/Participant against the risk of death or Total Permanent Disability due to any cause, as well as the risk of Dengue Fever during the Insurance Period and the Policy is active.
If the Insured / Participant suffers from Total Permanent Disability or dies due to illness or accident during the Insurance Period, 100% of the Sum Assured will be paid for the initial loan or a maximum of Rp. 100,000,000 and the amount is fixed.
If the Insured/Participant suffers from Dengue Fever during the Insurance Period, 100% of the Dengue Fever benefits will be paid at once in the amount of the initial loan or a maximum of Rp. 1,000,000. This benefit is only given once in a period of 1 Policy year and the policy will remain valid when the additional benefit of Dengue Fever has been paid.
Age 18 - 64 years (nearest birthday)
The monthly insurance period can be extended by making premium payments up to the age of the Insured/Participant who is 65 years old.

The premium is paid by the Insured/Participant through the Policyholder on a monthly basis to the Insurer's Bank account and must be paid within the Grace Period.
